THE APPLICATION FORM FOR INFORMATION ON PERSONAL DATA
1.THE SCOPE OF THE APPLICATION RIGHT
The owners of personal data (hereinafter will be referred to as the “Applicant”), which are defined as the relevant person in the Law on Protection of Personal Data No. 6698 (“PPD”), have been given the right to make certain requests regarding the processing of their personal data in accordance with the article 11 of the Law on PPD.
The rights of the applicant are as follows:
a. Finding out if your personal data is processed,
b. Requesting information if personal data is processed,
c. Learning the purpose of personal data processing and whether they are used in accordance with the purpose,
d. Learning third parties to whom personal data are transferred domestically or abroad,
e. Requesting correction if the personal data is incomplete or incorrectly processed and requesting notification to third parties to whom the personal data is transferred regarding the operations carried out within this context,
f. Requesting deletion, destruction or anonymization of the personal data if the reasons requiring its processing are no longer available although it has been processed in accordance with the provisions of the Law on PPD and other relevant laws, and and requesting notification to third parties to whom the personal data is transferred regarding the operations carried out within this context,
g. Objecting to a result of the automated analysis of the processed data which is against the interest of the person,
h. Requesting recovery of a damage stemming from unlawful processing of the personal data.
2.APPLICATION METHOD
In accordance with the first paragraph of Article 13 of the Law on PPD; Applications to CORONITY, the responsible party for the data, about these rights, must be submitted following the application methods below or by other methods specified by the Board of Personal Data (“Board”). Applications in the written format to CORONITY will be made according to one of the methods in the table by printing out this form. There is no need to print this form for applications made via e-mail. The applicant should pay attention to the following explanations in written applications.

5. FINALIZING APPLICATIONS
When the Applicant files a request regarding personal data to CORONITY in writing and through the methods described within the scope of this application form, CORONITY will finalize it and send it to the Applicant electronically, within 30 days from the date the request reaches CORONITY.
In order to ensure the security of personal data and prevent illegal data sharing, CORONITY may request information from the Applicant for ID authentication. CORONITY may ask questions to the Applicant about the application to make sure that the application is finalized in accordance with the request. If the application also requires some cost, the Applicant may need to pay a fee specified in the tariff determined by the Board (Communiqué on the Procedures and Principles of Application to the Data Controller – Article 7).
If the information in the application submitted by the applicant within the scope of this form is not correct and up-to-date or the application is made in an unauthorized manner, CORONITY cannot be held responsible for the demands arising from the application.
